Helping local families for 35 years… facing it together

“In January, our board chose to focus 100% of our 35-year-old nonprofit’s efforts to help local families, and keep 100% of every penny in the communities we serve.

The cost of this disease is bankrupting local individuals financially, emotionally, and physically, and the money you donate or raise by participating in our events like Walk4ALZ allows us to help at no cost. We are also raising money to support innovative, ground-breaking research here in the Greater LA area. Our change lets us commit 20% of Walk4ALZ proceeds directly to research.

Additionally, we continue to advocate on behalf of increased funding for research and resources for caregivers locally, statewide, and federally… for 35 years we’ve been on the ground making a difference for people and the cause. Today our commitment to Greater Los Angeles remains stronger than ever and we hope your commitment to us with our new name and new look remains strong as well.

Let’s face it… a diagnosis of Alzheimer’s is devastating. So until there’s a prevention, treatment, or cure… let’s face it TOGETHER.

“San Dimas Community Hospital (SDCH) is located on the east end of the San Gabriel Valley, one mile off the 210, 57, and 10 freeways, in the city of San Dimas, with a population of 35,000. San Dimas is a small, caring, cohesive community reaching from Puddingstone Lake in Bonelli Park on the south to the Foothills on the north. The hospital’s 13-acre campus consists of the 101-bed general acute care facility and two identical Medical Office Buildings. San Dimas Community Hospital draws the majority of its patients from its own town as well as the surrounding communities of Glendora, La Verne, Covina, West Covina, Azusa, Walnut, Diamond Bar, Pomona and Claremont. San Dimas Community Hospital is situated in a heavily penetrated managed care environment.
San Dimas Community Hospital opened its doors in 1971 as a physician-owned facility. At that time, the hospital was licensed for 99 beds. Patients and visitors often expressed their sense of the hospital’s “family” atmosphere. This was due not only to the efficient size of the facility, but more importantly because of the personalized care given by our physicians and employees, whether they are a nurse, housekeeper or a member of the dietary staff. In 2008, the hospital was sold to Prime Healthcare Services and operates as a 101-bed general acute care facility.”

“Since 1989 the USC Family Caregiver Support Center at USC Davis School has placed a high priority on providing support across the continuum of caregiving, including diagnosing, prognosis, services that help maintain the care recipient’s independence and abilities, helping caregivers care for themselves and manage their own well-being. Supportive services include information, assessment, individual consultations, respite, education, and training.”


Their vision is:

“To be a model for Los Angeles, the state and the nation, by using technology, research, and education to transform caregiving and the long-term care system.”

“If you live in LA county and help a relative or friend (age 60 and older) with one or more of the following tasks: physical care (getting in and out of bed), making health care decisions, cooking, cleaning, yard work, managing medications and going to doctor visits, making important decisions, Shopping for groceries and other items, legal issues, helping  with bill paying, Bathing and dressing, Providing transportation, Long-distance, caregiving. 

They offer Help to caring family and friends for all cultures and communities in LA county. They are funded in part by LA Community and Senior Services and the USC Davis School of Gerontology. Services are low or no-cost to family caregivers and friends who help someone over the age of 60.

At Care4You, San Dimas, CA we proudly care for our Veterans, their spouses and surviving spouses every day. Our commitment to their care is just one of the ways we demonstrate appreciation for their service to our country.

“1 in 3 seniors in this country is a veteran or surviving spouse of a veteran. These heroes – our fathers, mothers, spouses, and friends – may be eligible for a unique benefit through the VA called “Pension with Aid and Attendance.” This benefit may provide reimbursement funding of up to $25,440 a year to help cover medical expenses, including homecare services. Homecare helps keep loved ones safe and comfortable at home.”


“Millions of qualifying veterans and families go without Aid and Attendance simply because of not knowing about the benefit and inability to pay for homecare. Through our partnership with Veterans Care Coordination, we are able to assist qualifying veterans and their spouses to apply for and obtain the Aid and Attendance Benefit through the Department of Veterans Affairs. What makes this partnership unique is the fact that Veterans Care Coordination can offer financial assistance to clients that cannot afford to pay for the up-front, out of pocket expense of homecare through their Veterans Appreciation Program. Who’s Eligible?

 Veteran, or surviving spouse of a veteran, who served 90 consecutive days of active duty with at least 1 day during a wartime period …(other war eligibility may be required depending on when you served)

 Veteran/surviving spouse must have non-service connected medical condition which is not related to military service

 Veteran/surviving spouse must require assistance with daily activities, such as bathing, dressing, eating, medication management and personal care

  Veteran/surviving spouse must meet asset requirements based on age and marital (excludes home and car)

 Veteran/surviving spouse must meet the Medical Expense-to-Income Ratio.”

Couple: $2,120.00 Monthly

Veteran: $1,788.00 Monthly

Surviving Spouse:  $1,149.00 Monthly

Healthy Veteran with Sick Spouse: $1,404.00 Monthly
